EARLY ELICITATION OF ANTIBODIES IgG TO HELICOBACTER PYLORI INFECTION IN PATIENTS WITH MULTIPLE MYELOMA

Currently, the Kyrgyz Republic (Kirgizia) occupies one of the first places by frequency of elicitation in terms of the incidence of gastric cancer in oncological pathology among the Countries of Independent States.

As in both developed and in developing countries, Helicobacter pylori infection have ubiquitous propagation and leads to the development of erosive gastritis, gastric and duodenal ulcers, MALT-type lymphomas and play an important role at oncohematological diseases.

Long courses reception preparations, lead complement to the defeat of the gastrointestinal tract.

In connection, than the purpose of our study was the early elicitation of antibodies IgG to Helicobacter pylori infection at patients with multiple myeloma.

MATERIALS AND METHODS:

In the study group from March 2014 to September 2018 years, included 29 patients with different stages of the flow and immunochemical variants of the disease of multiple myeloma(MM). Of them kirgiz nationality- 9 patients (women-4, men-5), and Russian-speaking (multinational) population of the residents of Kirgizia-20 patients (women-8, men 12), aged from38 to 78 years

The study was conducted on the basis of the Eurasian Center for Oncohematology, Immunology and Therapy, in the laboratory of Pasteur Saint-Petersburg Research Institute of Epidemiologi and Microbiologi and the National Center of Oncology and Hematology, in laboratories of polyclinics in city Bishkek.

Further research on Helicobacter pylori infection in patients with multiple myeloma(MM) to continues.

DETERMINATION OF THE AMOUNT ANTIBODIES OF IgG TO INFECTION H.PILORI:

In order to determine the amount of antibodies IgG to Helicobacter pylori by enzyme immunoassay, analysis expressed in international units, was used in this test-system"ImmunoKombII H. pylori Ig G». The results were evaluated on a scale attached to the test system. On antibody levels serum distributed by the respective groups. 0-19.U /ml- negative, 20-39 U /ml- a low level of antibodies,40-90 U / ml - the average (middle) levels of antibodies, 91-120 or more U /ml- high levels of antibodies.

RESULTS AND DISCUSSION:

At conducted us research on the detection of IgG antibodies to H. pylori infection the first diagnosed patients with multiple myeloma, at detailed, thorough conversation-polls had in anamnesis of chronic pathologies of the stomach in remission, but clinical manifestations were noted in the form of hungry, nocturnal pains in the epigastric region and at reception rude food, and dyspeptic manifestations.

High, medium, low, and also negative levels of Ig G antibodies to H. pylori infection were detected in patients with MM as a result of examinations.

After the diagnosis of MM is established, selection of therapy is necessary and patients receive, depending on the stage of the disease, from the immunochemical variant and the severity of the condition as VAD therapy, or R-COP therapy, or immunomodulating drugs, or histonedeacetylase inhibitors, or proteasome inhibitors, or monoclonal antibodies.

With long courses of medication, non-tolerability of tablet preparations may develop drug gastritis, steroid ulcers in connection with which shown intravenous of drugs with curative and prophylactic purposes is indicated.

In patients with detection of IgG antibodies to Helicobacter pylori infection and in the anamnesis of gastrointestinal pathology as (erosive gastritis, gastric ulcer and duodenal ulcer) and the possibility of exacerbation and complications (ulcerative bleeding, secondary anemia, perforation, penetration), is necessary in parallel, simultaneously assign antibacterial therapy in combination with H2-blockers (De-nol, ranitidine, famotidine), proton pump inhibitors (esomeprazole, omeprazole), antacids (maalox, phosphalugel, almogel).

Thus, in the treatment of the underlying disease and in order to achieve a good effect from the performed polychemotherapy, autologous transplantation of the bone marrow and increase in the life expectancy of patients, it is necessary to simultaneously study and treat multiple myeloma patients with pathology of the gastrointestinal tract.

CONCLUSION:

1. Patients multiple myeloma is necessary include in scroll research on definition of Ig G antibodies to infection Helicobacter pylori.

2. In the presence of gG -antibodies to H.pylori infection at patients with multiple myeloma is necessary need to do a gastroscopy the with taking from different maximum up to 5 ulcerated sites of biopsy material and consultation of a gastroenterologist (oncologist according to indications).

3. In the treatment of the underlying disease (multiple myeloma) and in the presence of high, or medium, or low levels of IgG antibodies to H. pylori infection, is necessary prescribe concurrent antibiotic therapy in combination with H2 blockers, proton pump inhibitors, antacids.
